- Financial statement forms to be completed by each party and filed with the court in all cases where money or support is an issue. There are two forms, one for income of $75,000 or less and one for income of more than $75,000. In addition, if there is income from self-employment or a business owned by a party, you will need Schedule A and if there is rental income, you will need Schedule B.
